Key Responsibilities
- Deliver primary care services to short-term and long-term patients in post-acute facilities
- Diagnose acute illnesses and manage ongoing chronic medical conditions

Essential
- Active and unrestricted Nurse Practitioner license in the state of New York
- Valid DEA registration with full prescriptive authority
- Minimum of one year of clinical experience in a primary care environment
- Ability to work a full-time schedule of 5 days per week

- Previous experience working in post-acute care or skilled nursing facilities
- Experience managing complex chronic conditions in geriatric populations
