Key Responsibilities
- Deliver primary care services to short-term and long-term patients in post-acute facilities
- Diagnose acute illnesses and manage ongoing chronic medical conditions
- Formulate and execute comprehensive plans of care for a diverse patient panel
- Coordinate with physicians and facility staff to ensure seamless treatment delivery
- Counsel patients and families on lifestyle changes and coping strategies for aging
Essential
- Active and unrestricted Nurse Practitioner license in the state of New York
- Valid DEA registration with full prescriptive authority
- Minimum of one year of clinical experience in a primary care environment
- Ability to work a full-time schedule of 5 days per week
Preferred
- Previous experience working in post-acute care or skilled nursing facilities
